Why context counts as clinical information
Sleep, stress, money, work, caring, past experiences and how safe a body feels all shape what is realistic. Advice that ignores them tends to fail quietly and then get recorded as non-adherence. Treating context as part of the assessment usually explains more than repeating the advice does.
